This Federally Qualified Health Center (FQHC) serves a diverse patient population and offers the opportunity to practice full-scope primary care in a collaborative and supportive environment.

• Provide comprehensive primary care services to patients across the lifespan

• Full-scope family practice, including chronic disease management, preventive care, and acute visits

• May deliver off-site care as needed (e.g., home visits for homebound patients)
