We are currently seeking a dedicated and experienced Adult-Geriatric Nurse Practitioner or Physician Assistant to join a mission-driven primary care team in San Rafael, CA. About the Role:
As part of a well-established Federally Qualified Health Center network (previously serving with one of the largest community clinic organizations in California), you will provide comprehensive primary care to adults aged 18 and older, including geriatric patients. The clinic emphasizes team-based care and a strong collaborative culture.

Key Responsibilities:

  • Provide direct patient care including physical exams, diagnosis, treatment, and follow-up for adults and seniors (ages 18–100+).

  • Prescribe medications and develop treatment plans based on patient needs and clinical assessments.

  • Refer patients to specialists and collaborate with physicians, behavioral health providers, and support staff as needed.

  • Maintain accurate and timely documentation in accordance with organizational standards.

  • Participate in care team functions under the Patient-Centered Medical Home (PCMH) model.

  • Adhere to clinic policies, quality improvement goals, and performance metrics.

  • Maintain strong professional relationships with staff and patients, supporting a respectful and inclusive care environment.

  • Perform additional duties as directed by the Medical Director.

Qualifications:

  • Licensed NP (Adult-Geri or Family with strong adult/geriatric focus) or PA in the state of California.

  • Minimum of 3 years of clinical experience in a primary care setting.

  • Experience in community health, underserved populations, or FQHCs strongly preferred.

Compensation & Benefits:

  • Competitive salary range: $135,000–$160,000 annually

  • Signing bonus: Up to $50,000 for eligible applicants (program-specific criteria apply)
