Primary Care Advanced Practice Provider (APP)
Location: Mt. Carroll, IL

About the Position:
Hospital group is seeking a dedicated and patient-focused Advanced Practice Provider (APP) to join our busy outpatient clinic located in Mt. Carroll, Illinois.

• APP experience preferred

• Clinic-based role in a rural, community-focused setting

• Busy practice environment with an established patient panel

• Schedule: 4.5 days per week

• Current team: 1 experienced APP on staff

• Call: Some after-hours phone-only call required (minimal volume)

Compensation & Benefits:

• Salary: Competitive base range of $120,000–$180,000, plus production-based bonus potential
